General Information Regarding Payment:

Sessions are generally 50 minutes long, and are $125.00.   Payment is due at time of service.  At this time, I am only accepting privately paying clients.  

Acceptable forms of payment are Cash, Check, or Credit Cards (Visa, Mastercard, American Express, or Discover).  Payment is due in full at time of service. 

Initial phone consultation available free of charge. 

Insurance Information:
Services may be covered in full or in part by your health insurance.
You can check with your insurance carrier by asking them:
-Do I have mental health benefits?
-What is my deductible? Has it been met?
-How many sessions per calendar year does my plan cover?
-How much will you cover for an out-of-network provider?
-What is the coverage amount per therapy session?
-Do I need approval from my PCP?
You can also utilize a medical savings plan, if you have one.

Cancellation Policy:

Cancellations must occur 24 hours before the scheduled appointment time.  Missed appointments and cancellations that are made less than 24 hours before your appointment will result in a full billing of the scheduled time.

 Your First Appointment:

Once your first appointment is made, please take the time to fill out this intake form and bring it with you.  I will also go over my privacy policy and confidentiality policy with you at the time of your appointment.
