General Information Regarding Payment:

Sessions are generally 50 minutes long, and are $125.00.   Payment is due at time of service.  At this time, I am accepting both insurance and privately paying clients 

Acceptable forms of payment are Cash, Check, or Credit Cards (Visa, MasterCard, American Express, or Discover).  Payment or approved Co-payments are due in full at time of service. 

Initial consultation available free of charge.

I am currently able to accept several insurances as listed below. Additionally, just because your insurance is not listed here does not mean that they will not cover my services.  See below for more info on questions to ask your insurance company.

Services may be covered in full or in part by your health insurance even if it is not listed above.
You can check with your insurance carrier by asking them:
-Do I have mental health benefits?
-What is my deductible? Has it been met?
-How many sessions per calendar year does my plan cover?
-How much will you cover for an out-of-network provider?
-What is the coverage amount per therapy session?
-Do I need approval from my PCP?
You can also utilize a medical savings plan, if you have one.

Cancellation Policy:

Cancellations must occur 24 hours before the scheduled appointment time.  Missed appointments and cancellations that are made less than 24 hours before your appointment will result in a full billing of the scheduled time.

 Your First Appointment:

Once your first appointment is made, please take the time to fill out this intake form and bring it with you.  I will also go over my privacy policy and confidentiality policy with you at the time of your appointment.

About Medication:

 In holistic psychotherapy, medication is seen as an adjunct to whole health, as are many other methods of altering body chemistry - like yoga, nutrition, cardio exercise, acupuncture, and massage.  I work closely with several area psychiatrists and holistic health practitioners, and together we can decide if medication is the right tool for you at this time.
